<p><strong>Position Title</strong>: Telehealth Nurse</p><p><strong>Work Location</strong>: Remote work from home – with planned in office training and meetings in <strong>The Woodlands, TX</strong></p><p><strong>Assignment Duration</strong>: 6 months</p><p><strong>Work Schedule</strong>: Mon-Fri 7am-4pm CST</p><p><strong>Work Arrangement</strong>: Remote</p><p><strong>Position Summary:</strong></p><p>The Remote Oncology Triage Nurse provides telephonic triage services, assessing patient needs and collaborating with qualified staff to ensure appropriate care.</p><p>This role involves documenting all care elements in the patient's medical record and accurately identifying high-risk conditions, providing patient education and directing additional care to appropriate resources if necessary.</p><p><strong>Background & Context:</strong></p><p>Under general supervision, the nurse delivers professional nursing care adhering to national and organizational standards and guidelines, as well as state licensing board scope of practice. The candidate must recognize the physical, psychological, and spiritual aspects of patient care and participate in company-wide quality initiatives. This pivotal role supports the organizational Compliance Program, including the Code of Ethics and Business Standards, to ensure patients receive timely and expert guidance, enhancing patient outcomes and satisfaction.</p><p><strong>Key Responsibilities:</strong></p><p>• Respond promptly to patient calls, portal messages, and emails, using open-ended questions and standing orders to accurately assess and manage a wide range of symptoms and side effects.</p><p>• Evaluate the urgency of patient symptoms and manage them proactively, coordinating with other members of the healthcare team through electronic communication or by phone for cases that require urgent care beyond standard protocols.</p><p>• Conduct telephonic assessments of oncology patients, determining the necessary level of care and facilitating outpatient and/or telehealth visits when symptoms can be managed in that setting.</p><p>• Educate patients on symptom management, medication side effects, and treatment plans, providing clear instructions and empathetic support to enhance their understanding and compliance.</p><p>• Collaborate with other members of the healthcare team to ensure comprehensive patient care, documenting all interactions meticulously in the electronic health record (EHR) system.</p><p>• Utilize and adapt quickly to various technology and reporting platforms to ensure efficient and accurate remote patient care and documentation.</p><p>• Identify potential barriers to care, assisting patients in navigating healthcare systems and resources.</p><p>• Participate in ongoing professional development and stay current with best practices in oncology nursing and telephonic triage.</p><p><strong>Qualification & Experience:</strong></p><p>• Working hours of 7am-4pm Central time, Monday – Friday.</p><p>• Active and unrestricted Registered Nurse (RN) Texas multistate or other compact state license (additional state licenses may be required).</p><p>• Oncology Certified Nurse (or required to obtain within 1 year).</p><p>• Minimum of 3 years of clinical nursing experience, with at least 2 recent years in oncology (Hem-Onc clinic or hospital experience specifically).</p><p>• Excellent communication and interpersonal skills.</p><p>• Strong critical thinking and problem-solving abilities.</p><p>• Proficiency in using electronic health record systems and telecommunication tools.</p><p>• Ability to work independently and manage time effectively in a remote environment.</p><p>• Compassionate, patient-centered approach to care.</p><p>• Attend all onsite meetings and trainings located in The Woodlands, Tx office.</p><p>• Clinical assessment and critical thinking.</p><p>• Oncology symptom management and patient prioritization.</p><p>• Strong verbal and written communication.</p><p>• Patient education and care coordination.</p><p>• Accurate EMR documentation.</p><p>• Time management and organization.</p><p>• Ability to work independently in a remote environment.</p><p>• Proficiency with EMR systems and Microsoft Office applications.</p><p>• Ability to manage multiple priorities in a fast-paced setting.</p><p>• Bachelor’s degree in Nursing (BSN) preferred.</p><p>• Experience in telephonic triage or remote patient care is strongly preferred.</p><p><strong>Additional Information (If Applicable):</strong></p><p>This job description outlines the primary duties and qualifications required for the role of a Remote Telephonic Oncology Triage Nurse.
