Remote Medical Records Data Entry Assistant

, you will be the guardian of that integrity—ensuring that every piece of clinical information is accurately captured, meticulously organized, and securely transmitted. Your work will directly support field care teams, clinicians, and health‑system partners, allowing them to focus on delivering top‑tier patient care while you safeguard the data that makes it possible.

Key Responsibilities

    Data Accuracy & Compliance:

    Review, verify, and process patient charts to ensure completeness and adherence to HIPAA, company privacy policies, and industry‑standard classification systems.

      Medical Record Transmission:

      Coordinate the secure exchange of medical records with provider offices, hospitals, nephrology clinics, home‑health agencies, and other clinical entities.

        Scanning & Indexing:

        Accurately scan, index, and upload medical documents into the care‑management platform, maintaining a logical and searchable repository.

          Communication Management:

          Handle inbound and outbound communications—emails, calls, and portal messages—with professionalism and timeliness, serving as a liaison between care teams, providers, and patients.

            Data Entry & Verification:

            Enter, review, and validate member and provider information, ensuring that demographic and clinical data are error‑free and up‑to‑date.

              Support Care Plans:

              Perform administrative duties related to patient and provider care plan delivery, including documentation of admission and discharge status.

                Relationship Building:

                Establish and nurture positive, supportive relationships with providers, patients, and field teams to facilitate seamless clinical workflows.

                  Team Collaboration:

                  Participate in virtual meetings, training sessions, and cross‑functional projects as requested, contributing insights and best practices.

                    Continuous Improvement:

                    Identify opportunities for process enhancements, suggest workflow optimizations, and assist in the implementation of new data‑management tools.

                      Ad Hoc Duties:

                      Essential Qualifications

                      • High school diploma or GED equivalent (required).
                      • Minimum of one year of hands‑on experience handling medical records within a health‑care setting.
                      • Proficiency with Microsoft Office Suite (Word, Excel, Outlook, PowerPoint) and basic computer literacy, including the ability to scan, organize, and retrieve electronic health records.
                      • Demonstrated strong data‑entry skills with a keen eye for detail, ensuring high levels of accuracy and consistency.
