WE’RE HIRING: CLINICAL & NON-CLINICAL INTAKE RN

Are you an RN with hospital experience who thrives in a fast-paced environment, loves solving problems, and knows how to turn complex referrals into smooth patient admissions? We want you on our team!

We’re looking for a Clinical & Non-Clinical Intake RN to help coordinate home health and hospice referrals, ensuring every patient receives the right care at the right time.

What You’ll Do

  • Assess referrals and review clinical documentation for home health & hospice eligibility.
  • Verify insurance benefits and coordinate authorizations
  • Create and manage patient charts in MatrixCare
  • Ensure medical records and admission documentation are complete and accurate
  • Coordinate with physicians, RN supervisors, case managers, clinicians, social workers, marketers, and scheduling teams
  • Conduct patient/family welcome calls and answer SOC admission questions
  • Monitor hospitalizations and transitions of care
  • Navigate referral and insurance portals including Epic, CarePort, Allscripts, Navi, Ensocare, Availity, UHC, NGSConnex, CT DSS, and PointClickCare
  • Follow up on missing documentation and keep referrals moving
  • Support compliance, quality assurance, workflow improvement, and cross-coverage during high-volume periods

What We’re Looking For

  • Registered Nurse (RN)
  • 1+ year of hospital experience required
  • Healthcare intake coordination experience preferred
  • Home health and/or hospice experience is a strong plus
  • Knowledge of medical documentation, insurance verification, referrals, and authorizations
  • EMR and healthcare portal experience
  • Excellent communication, organization, multitasking, and teamwork skills
  • Commitment to HIPAA, CMS, state, and payer compliance
