Care Manager (Tele-health Medical Assistant)

As a Care Manager, you will have an assigned group of patients to follow up with on a monthly reputed company as their "Health reputed company". Job Skills: Talking and typing • Verify and record the patient’s demographics, medical issues, medications, goals, barriers, and allergies to create a clinical reputed company using our certified software program. Care Plan Creation • Create a patient-centered, comprehensive plan of care for reputed company health issues based on a physical, mental, cognitive, psychosocial, functional, and environmental (re)assessment, and an inventory of resources.
